[CHM 111] Printed Lab Report Cover Pages

Some of you had difficulty printing out lab report cover pages last 
week.  Others just didn't include them on your lab (and lost points).    
I have printed out some extra lab report form cover pages and placed 
them on the small table outside my office door in Willet (room 105).
