  There appears to be a typo in the book.  For the pz orbital the term 
out front is

1/( 4 Sqrt[2 Pi])

Not

1/ (4 Sqrt[2] Pi)

If you use the latter you will be left with an additional pi in the 
denominator of your integrated result.
