Okay -- Jon asked about this so here's some help...

Following the example in the text, I calculated the average cycles/word 
for a 1% cache hit rate as...

(0.01)*(2) + (1.00-0.01)(100) = 99.02

Using the data from the example the...

(2e9)(2)/(99.02) = 40.4 MFLOPS


The 99% cache hit rate value is MUCH higher.
